Referrals

Aspire Women’s Health is a referral-based clinic. STI testing and treatment does not require a referral.

For referring physicians

  • Fax referrals to (905) 619-1915.
  • No special form is needed: a short note with the inquiry or service requested is enough.
  • If you are part of an FHO or FHT, please say so on the referral to avoid outside-use penalties.
  • A CPP (cumulative patient profile) is appreciated.
